From the couch to the circle: group-analytic psychotherapy in practice

J Schlapobersky (2016)

Handbook of group therapy and a guide to the group-analytic model - the prevailing form of group therapy in Europe. It draws on the author's engagement as a practitioner and on the words and experience of people in groups as they face psychotherapy's key challenges - understanding and change. This book provides a manual of practice for therapists' use that includes detailed descriptions of groups at work; accounts of therapists' own experience and the issues they face in themselves and in their groups. It is devoted to the Group-Analytic model and brings the other psychodynamic models of group therapy - the Tavistock, Interpersonal, Psychodynamic, Modern Analytic and Systemic - into a comparative discussion creating an integrated and coherent approach. Divided into three sections: Foundations - aimed at practitioners using groups of any kind and working at every level including those providing supportive psychotherapy and groups for psychosis, trauma, those at risk, older people and children; The Group-Analytic Model - defines the group-analytic model at a basic and advanced level; and The Dynamics of Change - aimed at group analysts, psychotherapists and psychologists providing short-term psychotherapy and long-term group analysis. The book is illustrated with clinical vignettes and commentaries to explain the concepts in use. It is intended for those seeking psychotherapy, whether to resolve personal problems or to find new sources of meaning in their lives. It is also intended for policy-makers in mental health, teachers and students of different models of psychotherapy and the psychosocial field. The comparative discussion running through the text about methods and models of practice will be of interest to the wider mental health and psychotherapy fields. References / bibliography cited to individual chapters, has an index.

The author draws together the inherited wisdom of group analysis since Foulkes' time and makes his own lasting contribution. From the Couch to the Circle will be an invaluable, accessible resource for psychotherapists, psychoanalysts, psychologists, family therapists, academics, psychologists, mental health practitioners, academics and teachers in psychotherapy.

Staff Support Groups in the Helping Professions: Principles, Practice and Pitfalls

Edited by Phil Hartley, David Kennard

Staff burnout and work-related stress in mental health professionals cost the National Health Service not only millions of pounds each year, but also impact upon the welfare of those being cared for.

Staff Support Groups in the Helping Professions takes the lead from recent Department of Health initiatives, promoting the use of staff support groups to foster emotional resilience, deal with potential conflict and support reflective practice.

In this book Hartley, Kennard and their contributors explore the influences that help and hinder the setting up and running of staff support groups, and attempt to counter the often negative reactions that the term 'staff support' can evoke. They demonstrate that such support groups can be a sophisticated and valuable intervention that needs careful preparation and skilful management to succeed, and will in turn not only benefit the individual, but also the department as a whole and those that they care for.

Contributors share their experiences of facilitating support groups in a number of settings including:

* psychiatric wards

* therapeutic communities

* social services

* schools

* children's homes.

Containing a wealth of case material, Staff Support Groups in the Helping Professions will provide much-needed guidance for those professionals attending, managing, or in the process of setting up a staff support group.

The Psychodynamic Approach to Therapeutic Change

Michael Maltby and Rob Leiper

`It is well written and well organised and I'm sure it will be of help and interest to researchers and practitioners concerned with the therapeutic action of psychodynamic treatment' - Penelope Waite,Nurturing Potential

Change is the central purpose of all counselling and psychotherapy, but how it is conceptualized and worked with varies according to the theoretical approach being used. The Psychodynamic Approach toTherapeutic

Change explores the nature of psychological change from the psychodynamic perspective and describes the process through which clients can be helped to come to terms with painful experiences and develop new ways of relating.

In the first part of the book, Rob Leiper and Michael Maltby look at therapeutic change in relation to psychological health and maturity. They explore what motivates people to change and also why resistance occurs.

The main part of the book outlines the collaborative process that clients and therapist work through to bring about change and highlights the role of the therapist in:

Creating the conditions for clients to express their thoughts, feelings and memories

developing clients' awareness and understanding of their psychological processes, and

Providing `containment' for the client's psychological projections.

The final part of the book sets personal therapeutic change in a wider social context, linking individual change with community and organisational development.

Combining core psychodynamic concepts with contemporary thinking, The Psychodynamic Approach to Therapeutic Change provides a lively and up-to-date integration of ideas on the change process which will beof great value to trainees and practicing counsellors and psychotherapists.

Boys Who Have Abused

John Woods

John Woods presents a theoretical approach and practical suggestions for mental health practitioners working therapeutically with young people who have abused. Drawing on his long-standing experience, he has developed an integrated theory that bridges the gap between existing cognitive behavioural and psychoanalytic approaches. He shows how this individual treatment model can be applied in a range of contexts including residential settings, group and family work, as well as in individual work.

In-depth case studies throughout the book demonstrate how exploring the individual’s whole life-course within a psychoanalytic framework enables connections to be drawn between possible childhood abuse and subsequent abusive behaviour.

Guidelines are presented on working with the problems of self-destructiveness, masochism and depression facing the young abused/abuser and the impact of sexual abuse on sexuality, gender identity and sexual orientation. This is an instructiveand thought-provoking text for all mental health practitioners and allied professionals working with adolescents who sexually offend.

Strategic Management and Organisational Dynamics: The Challenge of Complexity

Ralph Stacey

In the fourth edition of this successful text, Ralph Stacey continues to question the view that organisations operate and succeed in relatively stable environments.

He argues that in order to succeed in uncertainty and continual change, organisations need to create new perspectives and learn from the chaos within which they operate.

This edition continues to focus on this radically different approach to strategic management.

The central tenets of this approach have to do with unpredictability and the limitations of control, and therefore it argues against the rational models of planning and control covered in other strategy textbooks. This is done by emphasising the importance of narrative, conversation and learning from one's own experience as the central means by which we can gain understanding and knowledge of strategy in organisations.

Complexity and Group Processes: A Radically Social Understanding of Individuals

Ralph D. Stacey

The increasing complexity of interdependence between people in modern life makes it more important than ever to understand processes of human relating.

In the West we tend to base our understanding of relating on the individual. "Complexity and Group Processes" suggests an alternative way of understanding human relating. The key questions covered in this book are: who am I and how have I come to be who I am?; who are we and how have we come to be who we are?; and how are we all changing, evolving and learning?

These are fundamental questions in the study of human interaction, and the answers explored in the book are relevant not only for therapeutic groups but also those who are managing, leading and working in organizations.

Race, Colour and the Processes of Racialisation

Farhad Dalal

Why should people of one colour come to hate and fear those of another? So many conflicts appear to be caused by racial and ethnic difference; for example, the cities of Britain and America are regularly affected by race riots. It is argued by socio-biologists and some schools of psychoanalysis that our instincts are programmed to hate those different to us by evolutionary and developmental mechanisms. This book argues against this line, proposing an alternative drawing on insights from diverse disciplines including psychoanalysis, sociology, social psychology and linguistics, to give power-relations a critical explanatory role in the generation of hatreds.

Farhad Dalal follows Norbert Elias to argue that the primary function of race is to make a distinction between the "haves" and "must-not-haves", and that this process is cognitive, emotional and political. Examining aspects of the subject over the past thousand years, Race, Colour and the Processes of Racialization describes how the world and mind were made black and white, covering:

• Psychoanalytic and other theories of racism

• A new theorization of racism based on group analytic theory

• A general theory of difference based on the works of Fanon, Elias, Matte-Blanco, Winnicott and Foulkes

• Application of this theory to race and racism

Farhad Dalal concludes that the structures of society are reflected in the structures of the psyche, and both of these are colour coded. This book will be invaluable to students, academics and practitioners in the areas of psychoanalysis, group analysis, sociology, psychotherapy and counselling.

Taking the Group Seriously

Farhad Dalal , foreword by Malcolm Pines

In this critique and extension of the work of S.H.Foulkes, Farhad Dalal presents a thorough contemporary appraisal of the theory of group analysis and its relevance to psychoanalysis as a whole. The author argues that Foulkes failed to develop a specific set of group concepts, relying instead on the traditional individualistic framework of Freud. The book explores why Foulkes failed to escape from the orthodox mother- infant paradigm and offers a new post-Foulkesian interpretation of group analytic theory.

Taking the Group Seriously is divided into six parts which trace the history of ideas behind group work, and draws on a wide range of subjects to support its thesis: not only psychoanalysis and group analysis, but also sociology, biology, chaos theory, genetics, economics, game theory and discourse theory. Using the author's practical group experience and including the latest ideas on the subject, this volume will be of interest to all those working in the field of psychoanalysis.

Traumatic Experience in the Unconscious Life of Groups

Earl Hopper , foreword by Malcolm Pines

Working within the traditions of Bion, Turquet, Foulkes and Pines, and drawing on concepts and data from psychoanalysis, group analysis and sociology, this volume develops Earl Hopper ’s new theory of the fourth basic assumption in the unconscious life of groups and group-like social systems. Located within a social, cultural and political transgenerational context, Incohesion: Aggregation/Massification or (ba) I:A/M (an acronym for ‘I AM’ – an assertion of personal identity when identity is under threat) is based on the fear of annihilation stemming from traumatic experience.

With full respect for the constraints of the social unconscious, Earl Hopper applies his theory of Incohesion to the treatment of ‘difficult’ patients in group analysis.

The personification of aggregation and massification – patients with crustacean, contact-shunning and amoeboid, merger-hungry characteristics respectively – is illustrated with detailed clinical vignettes from heterogeneous groups, including drug addicts, victims of incest and sexual abuse, and child survivors of the Shoah. Concluding with critical commentaries by senior British and American group analysts and psychoanalysts, this volume will make essential reading for both analysts and their students.

The Social Unconscious

Earl Hopper , foreword by Malcolm Pines

The social unconscious and its manifestations in group analysis are the focus of this important new book of Earl Hopper 's selected papers. Drawing on sociology, psychoanalysis and group analysis, he argues that groups and their participants are constrained unconsciously by social, cultural and political facts and forces. These hypotheses are illustrated with clinical vignettes concerning anti-Semitism, racism, the politics of class and gender, and the effects of rapid social change. Transference and countertransference processes are examined both vividly and honestly. Theoretically generative and clinically astute, this book will be of value to both analysts and their students.

The French Revolution - A Tale of Terror and Hope for Our Times
Harold Behr

This is the story of the French Revolution told from a psychological and group dynamic perspective. The aim is to throw light on the workings of the revolutionary mind and the emotions at work in society which pave the way towards revolution and war. Louis XVI and Marie Antoinette are presented as a couple trapped by the symbolism invested in them, a circumstance that turned them into scapegoats. The contrasting personalities of the two most controversial leaders of the Revolution – Robespierre and Danton – provide psychologically informed explanations of their success and failure as leaders.

The group perspective – the nature of crowd behaviour and mob violence – links to the complex relationship between leaders and groups. In the Parisian case of 1789 group emotions – fear, rage, euphoria and fervour – influenced the course of the Revolution. The assassination of Marat and the struggle to the death between the extremists of the Left and the Moderates is a classic study in group paranoia culminating in a Reign of Terror destined to end in self-destructive violence. The conflict between the Revolution and the Church as an expression of belief in an ideal society led to a battle for the minds of a people facing two incompatible ideologies.

Group Analytic Psychotherapy – A Meeting of Minds

Harold Behr & Liesel Hearst

His book offers practitioners, teachers and students of psychotherapy a detailed and comprehensive account of group analysis. It demystifies the workings of analytic groups and looks at the great stretch of issues and tasks confronting the therapist in the practice of group analytic psychotherapy. Each stage in the process is fully discussed: the assessment and preparation of patients for groups, dynamic administration, beginning and ending a group, and the introduction of new members into an established group. A chapter on psychopathology gives a picture of the main psychiatric conditions which the group therapist is likely to encounter, and offers clear guidelines on how to manage them in a group context. An exposition on the group in full flow provides an unusual insight into the processes which constitute the analytic culture, including the analysis of dreams, the art of interpreting, use of the transference and counter transference, and the place of play, humour and metaphor.

Difficult and challenging scenarios, such as dropping out, scapegoating, the silent group member, and monopolisation of the group are treated in depth, as are Large Groups, homogeneous groups, groups for children and adolescents, family therapy, groups in non-clinical settings, and the supervision of group therapy.

The impingement of the therapist’s own personal issues is also given attention. The authors have flanked their narrative with accounts of the historical, social and cultural origins of group analysis, and a vision of the future provided by the newer strands of thinking in the field. The text is enlivened by colourful vignettes drawn from the authors' own experiences, and by sharply focused dialogues between the two authors, designed to illustrate their contrasting and complementary perspectives. The book represents a distillation of the authors' long experience in the field of group analytic practice and training in the United Kingdom and internationally.

Psychotherapy and Counselling – A Professional Business

Cynthia Rogers

Cynthia Rogers is interested in the stresses a psychotherapist encounters over the course of his or her professional life and how these might be addressed. Running a practice that provides a living is complex in itself, but how do people manage when life events or their own insecurities intrude?

Therapists with thriving practices know that, at some stage, they will have to confront a complaint, suicide, a bad debt, a lack of referrals or deal with the impact of personal life events. These events are part of the territory and a way has to be found for professionals to take them in their stride however stressful they may be.

Psychotherapists traditionally learn from experience and Cynthia has distilled her own and others' experiences to throw some light on the shadow side of a psychotherapist' s life.

Experienced psychotherapists are emerging from the consulting room and becoming supervisors, trainers and consultants. They are also moving between private practice, GP surgeries and the voluntary sector. Cynthia Rogers describes the demands of each setting and shows how to draw on new parts of one' s self and learn new ways of working at each move.

The Group as an Object of Desire

Exploring Sexuality in Group Therapy

Morris Nitsun

In this original and ground-breaking book, Morris Nitsun argues that desire and sexuality are key components of human experience that have been marginalized in the group psychotherapy literature. Drawing on theory from psychoanalysis, developmental psychology and sociology, while keeping the group firmly in focus, he creates a picture of the potential in group therapy for the most intimate narrative.

Highlighting current concerns about sexual identity, boundary transgression and what constitutes effective psychotherapy, detailed clinical illustrations cover areas such as:

  • The erotic connection

  • The dissociation of desire

  • The group as witness

  • Erotic transference and counter-transference

Psychotherapists and all those interested in sexual development and diversity will value the challenging approach to sexuality this book offers.

Selected Papers by Dennis Brown: Resonance and Reciprocity Editor(s) - Jason Maratos

This collection of papers, published between 1976 and 2003, traces the innovative connections which the eminent group analyst Dennis Brown made between medicine and psychoanalysis.

They reveal his important insights into how the principles of group analysis can improve our understanding of philosophy and ethics, and trace the development of trans-cultural dimensions of group analysis.

Beginning with Dennis’ early work in dermatology, the first section of Resonance and Reciprocity provides a fascinating overview of the insights gained into psychosomatic conditions through the application of psychoanalysis and group analysis.

The second section builds on the tenet of group analysis that therapy should change the therapist as well as the client, addressing the changes that can take place in the therapeutic milieu, both in client and provider. The chapter on drowsiness, a modern classic, provides a significant contribution to our understanding of the emotional and physical changes that the therapist experiences during analysis and its wider implications for our appreciation of how changes in mental and physical states are affectedby a person’s emotional world.

The final section reveals how Dennis Brown extended his interest and his activities beyond the individual, the small and the large group, and studied groups within and across cultures.

This book provides not only a solid understanding of complex analytical notions but also opens the road for future development. It will appeal to students and professionals in psychoanalysis, psychotherapy and group psychotherapy.

How to be a Good Enough GP

Gerhard Wilke with Simon Freeman

The upheavals of the NHS reforms have caused a great deal of stress and uncertainty in primary care, and professional development and support for general practitioners needs to take account of this. This book offers a group supervision model which can be used to develop the core competencies needed for GPs to make the new primary care organisations work.

The book analyses how primary care professionals have dealt with the various reforms of the past decade, and picks apart the paralysing culture of politeness, conflict avoidance and rivalry for power, to reveal how at the core of reform is the struggle for each GP to construct a new professional identity which integrates medicine, management and politics. It proposes ways GPs can benefit from these experiences to become equipped with the necessary competencies to be active members or dynamic leaders in the new primary care organisations.

The doctor-patient relationship is no longer one-to-one, but located within a group matrix, in the same way that a GP is now required to work within a group framework. This book enables GPs to develop the essential group skills they now need, and on which the success of the healthcare reforms ultimately depends.

This book will be the first to focus exclusively on inpatient therapeutic groupwork in acute psychiatry, from a multi-disciplinary perspective. All authors are active groupwork practitioners, who provide vivid case material providing unique insights into the group process.

Writers make the argument for the importance of therapeutic groupwork in acute inpatient settings. They describes survey data that show an absence of therapeutic activity on wards and the need for a ‘culture of participation’. They describe some national schemes designed to improve the situation, such as the Star Wards initiative. Two authors look the state of research on therapeutic groupwork in inpatient settings, and suggest how the evidence base might be strengthened.

The book will be of great value to any mental health professional, whether qualified or in training. Although reflecting experience in British clinical settings, the issues raised have a wider interest for those working to achieve excellent acute inpatient psychiatric settings in other countries.

A Therapeutic Community Approach to Care in the Community

Edited by Sarah Tucker

Applying therapeutic community principles to community care, this book advocates the active engagement of clients in their own lives and promotes the development of a shared sense of responsibility between client and carer.

The contributors define care and dwelling as central to human life, and examine the psychological significance of having a place of one's own. They argue that the client should be encouraged to participate actively in the provision of these, rather than remaining a passive recipient, in order to gain a sense of being part of a social environment.

However, a dwelling is more than a physical space, and the fundamental importance of community, and of dialogue in creating that community, is also explored. The contributors provide a philosophical framework for these ideas, but they also discuss them in terms of the practicalities of a therapeutic community: how interactions can be developed and how the tensions of communal living can be defused.

Therapeutic Community Approach to Care in the Community focuses on the practical implications and use of dialogue, language and education in community care. A stimulating contribution to the debate on the future of community care, the book will interest all mental health professionals concerned with working with their clients in terms of their whole environment, not just in isolation.
